Final approval of all classes will be based on class size and individual student skill level. Registration fees will need to be paid to hold child’s “spot” in class(es).

By enrolling my child/myself in classes at Canyon Dance Academy, I hereby assume responsibility for injuries caused when regulations at the academy are ignored. These regulations include, but are not limited to, using the studio without supervision, misusing the equipment, participating in class without the proper footwear and disobeying instructions. I grant my child or ward permission to participate in Canyon Dance Academy’s 2022-2023 season. I hereby release and discharge Canyon Dance Academy, its agents, employees, and officers from all claims, demands, actions, judgments and execution with the undersigned heirs, executors, administers, or assign for all personal injuries caused by, or arising from, the above described activities related thereto. Further, I grant Canyon Dance Academy, its agents and employees’ permission to authorize any emergency medical treatment that may be required for my child or ward during the 2022-2023 season. I, the undersigned, have read this release and authorization and understand all its terms. I execute it voluntarily and with full knowledge of its significance. I have executed this release and authorization on the day and year stated below.

I, the undersigned, do hereby consent and agree that Canyon Dance Academy, its employees, or agents have the right to take photographs, videotape, or digital recordings of me beginning on September 6, 2022 and ending on September 6, 2023 and to use these in all media, now or hereafter known, and exclusively for advertisement. I further consent that my name and identity may be revealed therein or by descriptive text or commentary. I do hereby release to Canyon Dance Academy, its agents, and employees all rights to exhibit this work in print and electronic form publicly or privately and to market and sell copies. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used. I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or playback. I also understand that Canyon Dance Academy is not responsible for any expense or liability incurred because of my participation in this recording, including medical expenses due to any sickness or injury incurred as a result. I represent that I am at least 18 years of age, or legal guardian, have read and understand the foregoing statement, and am competent to execute this agreement.
